The College of Management forms listed below are now submitted online.  Paper versions of these forms are no longer available or accepted.

Forms for Non-CM Students:

• Undergraduate Registration Request for Non-CM students:  Available January 9th – January 30th for Non-CM students requesting registration in CM courses for Spring 2012.  Non-CM students may request registration in the following CM courses ONLY:
  AF 210, AF 301, BC 230, BC 290, MGT 303, MKT 301, MSIS 110, MSIS 212

Forms for CM Students:

• Concentration Declaration Form:  Available January 1st – March 1st for CM students with 60 credits earned and at least one semester completed in the College of Management to declare their concentration.

• Request for Credit Overload:  Available November 14th – January 30th for CM students requesting to register in more than 17 credits in Spring 2012.
  

• Undergraduate Registration Request:  Available November 14th – January 30th for CM undergraduate students to request registration in CM courses for Spring 2012.
